Facial Plastic Surgery Training and Experience
Training and Academic Medicine
Dr. Derakhshan completed an extensive course of medical and surgical training at several of the nation’s respected academic institutions:
- Doctor of Medicine: Cleveland Clinic Lerner College of Medicine
- Residency in Otolaryngology–Head and Neck Surgery: Massachusetts Eye and Ear / Harvard Medical School
- Fellowship in Facial Plastic and Reconstructive Surgery: Johns Hopkins University
- Assistant Professor: Loma Linda University
His background in otolaryngology–head and neck surgery provides a detailed understanding of facial anatomy, breathing, function, and the relationships among the structures of the face and neck. His fellowship training at Johns Hopkins further concentrated that knowledge on aesthetic and reconstructive facial surgery.
Alongside his private practice, Dr. Derakhshan remains involved in surgical education and research as an assistant professor at Loma Linda University. His continued participation in academic medicine helps him remain closely connected to evolving surgical knowledge while contributing to the training of future physicians.
Facial Plastic Surgery Procedures
Rhinoplasty
Rhinoplasty reshapes the nose to improve its relationship with the rest of the face. Depending on the patient’s needs, surgery may address the bridge, nasal tip, nostrils, overall size, asymmetry, or structural concerns that affect breathing.
Dr. Derakhshan plans rhinoplasty around the patient’s facial proportions rather than attempting to reproduce the same nose on every person.
Revision Rhinoplasty
Revision rhinoplasty addresses aesthetic or functional concerns that remain after a previous nasal operation. These cases can be more complex because scar tissue, altered anatomy, or limited structural support may affect the available surgical options.
Dr. Derakhshan evaluates the existing nasal framework, breathing function, prior changes, and the patient’s priorities before developing a revision plan.
Facelift and Neck Lift
Facelift and neck lift surgery addresses descended facial tissue, jowls, diminished jawline definition, loose neck skin, and visible neck bands.
Dr. Derakhshan generally uses a deep-plane approach so that underlying tissues can be released and repositioned rather than relying primarily on tension applied to the skin. Because the face and neck are anatomically continuous, he evaluates them together to create one harmonious result.
Eyelid Surgery
Eyelid surgery, or blepharoplasty, can address excess upper eyelid skin and puffiness or bags beneath the eyes. Treatment is customized to refresh the eye area while preserving natural expression, eyelid function, and the individual shape of the patient’s eyes.
Brow Lift
A brow lift can improve a descended or heavy brow and soften changes that contribute to a tired or aged appearance. Dr. Derakhshan considers the brow position relative to the forehead, eyelids, hairline, and overall facial proportions.
Lip Lift
A lip lift shortens the space between the base of the nose and the upper lip. In appropriately selected patients, it can increase the visible height of the upper lip, enhance its shape, and improve facial balance without adding excessive volume.
Ear Reshaping
Ear reshaping, or otoplasty, can improve prominent ears or concerns involving ear shape, proportion, or symmetry. Surgery is tailored to create a more balanced relationship between the ears, head, and face while maintaining a natural appearance.

Frequently Asked Questions
-
What does a facial plastic and reconstructive surgeon specialize in?
A facial plastic and reconstructive surgeon focuses on procedures involving the face, nose, eyes, ears, head, and neck. Dr. Derakhshan completed residency training in otolaryngology–head and neck surgery followed by fellowship training specifically in facial plastic and reconstructive surgery.
-
How does Dr. Derakhshan create natural-looking results?
Dr. Derakhshan plans surgery around the patient’s existing anatomy rather than applying predetermined proportions or trends. He considers the face a complete system, so the treated feature remains balanced with the features around it.
-
Can facial procedures be combined?
Yes. Depending on the patient’s anatomy, health, and goals, procedures such as a facelift and neck lift, eyelid surgery, or a brow lift may be performed together.
-
Am I too young or too old for facial rejuvenation surgery?
Age by itself does not determine candidacy. Dr. Derakhshan evaluates the condition and position of the facial tissues, the patient’s overall health, and whether surgery can produce a meaningful improvement.
-
What should I expect during my consultation?
Dr. Derakhshan will ask what you would like to address and what you hope to achieve. He will evaluate your anatomy, explain appropriate options, and discuss expected results, recovery, limitations, and risks.
